Title:Neuroform stent assisted transarterial coil embolization of basilar artery apex cerebral aneurysm in a 62-year-old male

Summary:
62-year-old Caucasian man with a 13 mm basilar tip aneurysm underwent a stent assisted transarterial coil embolization.

Presentation:
62-year-old Caucasian man with history of hypertension and coronary artery disease as well as peripheral vascular disease was found to have a 13 mm basilar tip aneurysm during evaluation for prior TIA and headache.

Procedure Detail:
An SL 10 microcatheter was advanced and placed over Synchro microwire and under fluoroscopic and road map guidance into basilar tip aneurysm and several GDC and matrix coils were deployed sequentially. Due to coils herniation into the proximal left P1 segment, the decision was made to protect the arteries with Y-stent configuration. Subsequently, the microcatheter was removed from the aneurysm lumen and was used to assist the exchange length microwire to access the right posterior cerebral artery. Subsequently, Neuroform-2 stent, 4 mm x 20 mm, was deployed across the aneurysm neck spanning between the proximal right P1 segment and the distal basilar artery. A second Neuroform-3 stent, 4.5 x 15 mm, was advanced through the first stent in Y-configuration fashion spanning between the left P1 segment and the distal basilar artery. The microcatheter was repositioned into the aneurysm lumen and additional coils were deployed for a total of 16 coils.
